SSM Health is investing in healthier communities through a $10,000 Community Health gift to Fond du Lac Literacy Services (FLS) to support its Health Literacy & Telehealth Access Through Adult Education program.
The initiative helps adults develop literacy, language, and digital skills needed to better access health care, understand medical information, and participate in telehealth services.
As the only organization in Fond du Lac County providing literacy instruction for adults reading below a sixth-grade level, Fond du Lac Literacy Services serves individuals who face barriers related to literacy, English language proficiency and digital access. Through free, individualized tutoring, learners gain practical skills that help them navigate health care systems, communicate with providers, and access patient portals and virtual care services.
“Health literacy is a critical part of overall health and well-being,” according to Laura Nakielski, SSM Health Community Health manager. “Many individuals face challenges understanding medical information, scheduling appointments or accessing telehealth services. By supporting Fond du Lac Literacy Services, we are helping remove those barriers and empowering community members with the skills and confidence they need to take charge of their health.”
The program aligns with Fond du Lac County Community Health Improvement Plan priorities by addressing health care access and health literacy among underserved adults. Participants receive instruction in literacy, English language learning, digital skills and telehealth navigation, helping them use healthcare resources more effectively and engage in preventive care and chronic disease management.
“We are deeply grateful to SSM Health for this generous investment in our learners and our community,” according to Juana Montes, Fond du Lac Literacy Services coordinator. “For many adults, improving literacy and digital skills opens the door to better health, greater independence and stronger opportunities for themselves and their families. This support will help us continue providing individualized instruction that enables learners to navigate healthcare systems, access telehealth services and advocate for their own well-being.”
Fond du Lac Literacy Services served 156 adult learners in 2025 and aims to serve 200 learners in 2026. Through the program, participants receive one-on-one tutoring and small-group instruction that incorporates health literacy and digital education. The organization also collaborates with community partners to connect learners with health care resources and services.
The SSM Health Community Health Giving Program supports local organizations that address priority health needs and improve the health and well-being of communities served by SSM Health. Through partnerships like this one, SSM Health continues to advance community-driven solutions that create healthier futures for individuals and families throughout Fond du Lac County.
