In response to the 2024 Community Health Assessment, three health priorities have been identified: Mental Health, Substance Use, and Healthcare Access.
To move forward in developing a plan to address priority areas, Fond du Lac County Health Department and SSM Health in collaboration with Healthy Fond du Lac County partners, are asking organizations and community members to participate in 3 separate planning discussions.
The purpose of the community health improvement plan and strategizing sessions is to facilitate a collaborative process for organizations to take collective action towards an identified community health priority and improve population health. The plan is more comprehensive than roles and responsibilities of a health department and health care systems alone and should be community-driven. The goal is to set priorities, select strategies, direct the use or resources, and develop and implement projects, programs, and policies with stakeholders and partners.
Each session will feature a presenter(s) who will review community-based goals, achievements, and challenges in each of the topics. Then, participants will engage in a strategizing session where we break into small workgroups to select a strategy for implementation and identify resources required, actions steps, timelines, and anticipated outcomes for a shared action plan. The action plans will then become adopted by the coalition ‘action team’ to work towards implementation.
