Strategizing the 2022-2024 Community Health Improvement Plan For Fond du Lac County

The Fond du Lac County Health Department and SSM Health are hosting community strategizing sessions to discuss community goals, strategies, and ways we can make a collective impact to improve the quality of life and address top health needs in our community. These sessions are free and open to the public. These discussions will inform the 2022-2024 Community Health Improvement Plan (CHIP). The CHIP is the action plan to address the health priorities determined in the most recent Community Health Assessment (CHA) Report: Mental Health, Chronic Disease, and Substance Use. “To have a significant impact on population health, community members and key leaders across all sectors, must work together collaboratively on a common agenda addressing the factors that shape health,” said Kim Mueller, Fond du Lac County Health Officer. The next steps to move our community forward include identifying strategies that will work for our community to create meaningful change.

We encourage local businesses, healthcare, non-profits, educators, government, community/service groups, residents, and more to attend these sessions and learn how they can be active partners in strategies identified.

Three separate sessions will be held, each focusing on one of the priorities. Attend one or all three:

  • Tuesday October 4, 2022 from 7:00am – 8:30am – Mental Health (Fond du Lac)
    • Location: Marian University Stayer Center (S National Avenue Fond du Lac, WI 54935)
  • Wednesday October 5, 2022 from 7:00am-8:30am – Chronic Disease (Fond du Lac)
    • Location: Marian University Stayer Center (S National Avenue Fond du Lac, WI 54935)
  • Thursday October 6, 2022 from 8:00am-9:30am – Substance Use (Ripon)
    • Location: Ripon College Pickard Commons (304 Elm Street Ripon, WI 54971)
